Sonoma Valley Hospital has partnered with Verge Health to offer an online affiliation verification service. This service provides instantaneous, online primary source verification of a practitioner’s affiliation with Sonoma Valley Hospital. The link to the credentials verification service is:

Please be advised that it is the policy of Sonoma Valley Hospital not to disclose to other health care institutions or practitioners any disciplinary actions, investigations, or similar actions regarding practitioners affiliated with Sonoma Valley Hospital. Sonoma Valley Hospital participates in the National Practitioner Data Bank established by the Health Care Quality Improvement Act of 1986 and files reports on credentialed staff members as required by law and regulations governing the data bank.
