MyChart

MyChart is a secure online portal that allows you to access your SVH electronic medical record and send messages to your doctor directly without going to the hospital or picking up the phone.

Activation codes will be sent to SVH patients that have given consent to receive notifications by email or in a text beginning December 3, 2022. After December 3, patients will be able to register. Those patients that already are using MyChart will be able to link the SVH MyChart to theirs.

Beginning December 3, patients with questions about MyChart can call the MyChart Patient Engagement Team at 1-833-395-2035 Monday – Friday: 7:00 am – 7:00 pm and Saturday – Sunday: 9:00 am – 5:00 pm. Interpretation services will be made available.

MyChart offers many advantages – the most important is that your SVH treatment records will reside all in one place for our doctors and other medical professionals providing care beginning December 3.

Your SVH medical information will remain confidential, billing will be simplified, lab and medical imaging results easy to access, and available 24/7.

Patients will be able to access health information such as summaries of recent visits, and test results – day or night.
