Sonoma Valley Hospital Honored by Partnership HealthPlan of California for High Quality

February 26, 2025 — Sonoma Valley Hospital (SVH) is excited to announce recognition as a top performing hospital by Partnership HealthPlan’s (PHP) Hospital Quality Improve Program (HQIP).
Sonoma Valley Hospital is one of only 8 hospitals that earned a perfect 100% score. This includes participating hospitals from Butte, Colusa, Del Norte. Glenn, Humboldt, Lake, Lassen, Marin, Mendocino, Modoc, Napa, Nevada, Placer, Plumas, Shasta, Sierra, Siskiyou, Solano, Sonoma, Sutter, Tehama, Trinity,Yolo, and Yuba Counties.

The goal of the HQIP is to improve the quality of care provided to PHP members. Measures are selected and designed in collaboration with hospital partners and represent the following domains: readmissions, advance care planning, clinical quality, patient safety, and operations and efficiency.
Partnership HealthPlan of California is a community-based, county-organized health system plan that provides health care delivery to over 900,000 low-income Medi-Cal recipients. Partnership’s goals are to improve access, quality, and cost effectiveness through a managed care system. Partnership links members with a primary care provider and has been successful in reducing inappropriate use of emergency rooms, providing suitable levels of inpatient care, developing innovative case management programs, and providing services more locally.
Partnership began operations in May 1994, and was developed through a community coalition formed to address the problem of health care access for Solano County’s growing number of medically uninsured and underserved. Partnership has seen several geographic expansions over the years and now serves members in 24 Northern California counties.
